What type; what for; how long ago; how was it?
8) Are you being treated for a current medical diagnosis?
E.g. epilepsy, asthma, psychosis, pace maker, mental wellbeing?
9) Have you had recent serious thoughts about hurting yourself or others? 10) Are you on any medication or drugs? 11) How much do you want to make this change? (1=not fussed. 10=most definite.)
